Health Requirements are in the table below. Requirements are for ALL Kindergarten students, ALL second grade students and ALL students new to the district (must provide the same documents as Kindergarten).
These documents cannot be scanned in to the online registration system, the school office must be provided a copy.
Physicals and Immunizations are due by the first day of school, August 22.
