*All student athletes must have a valid athletic physical provided in the last 12-13 months to participate in interscholastic sports.

Participation Requirements
All student-athletes and their parents must complete a District 186 Athletic Permit Card prior to going out for a sport or activity.
All high school students shall be assessed a fee of $85 for participation in all extra-curricular activities governed by the IHSA. Waiver forms may be found in the main office.
Athletes participating in all sports must have record of insurance or an insurance waiver signed by the parents/guardian.
